29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents: Employees were exposed to struck-by hazards. Employees installing tyvek insulation at the commercial construction site were exposed to eye injury in that employees were using pneumatic staple guns and were not provided with eye protection.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Employees were exposed to fall hazards. An employee installing Tyvek Home Wrap on a commercial building was exposed to a fall distance of approximately 20 feet and was not tied off to an anchorage point in the basket of the aerial lift to prevent a fall.
