29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy. On 5/18/18, at the Cumming facility, the Lockout/Tagout procedures implemented at the facility were not in a language all Authorized employees could understand. i.e. Burmese employees who were deemed Lockout Authorized employees did not speak or read English Lockout and were provided procedures that were in English only.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On 5/18/2018, at the Cumming facility, the employer did not develop a specific lockout procedure for the Finishing Chiller pump motor. A sanitation employee received an amputation as he cleaned the Finishing Chiller pump motor without the equipment being locked out. Tyson Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1029958, citation number 1, item number 1 and was affirmed as a final order on 7/6/2015, with respect to a workplace located at 5807 Mitchell Ave., Saint Joseph, MO. Tyson Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1014235, citation number 1, item number 3 and was affirmed as a final order on 10/5/2015, with respect to a workplace located at 400 West Olrich St. Rogers, AR.
