(a) Robinson Campbell Pad- 1198 Campbell Road, Bulger, PA: On or about May 29, 2018, employees were exposed to crushed by hazards while operating a Moffett forklift across an incline; the forklift tipped over, crushing the employees legs and requiring multiple surgeries.
