29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a)  Worksite, north side of building - The employee was exposed to falling approximately 18 feet to the ground below. The scaffold had one plank installed on the third tier; on or about 5/30/18.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:   a)  Worksite - Employees were exposed to falling while working from the third tier of a fabricated frame scaffold, approximately 18ft above the ground; on or about 5/30/18.     Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.451(f)(6):   Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.   a)  Worksite - Employees were framing a chimney on the exterior of a residential home while working from a fabricated frame scaffold.  The scaffold was set up next to the energized power lines entering the home; on or about 5/30/18.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
