29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a. On May 18, 2018 where an employee was operating a bottom seal bag machines, the employee was not adequately protected from the rotating parts of the machine, resulting in a severe injury to an employee's hand requiring overnight hospitalization and surgery.  b. On or before May 29, 2018 where employees operated bottom seal bag machines, the employees were not adequately protected from the rotating parts of the machine, exposing employees to caught-in hazards.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.    For the calendar year 2018 OSHA 300 log, the employer had not included all the information in column F as required for the following entries:    a. Case no. RCL-17-1 did not describe the injury or which part of the body that was affected.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a. On or before May 19, 2018, the employer failed to report the overnight hospitalization of an employee as a result of a workplace incident which occurred on May 18, 2018 at approximately 10:30 pm which resulted in a severe injury to the employee's right hand.
