29 CFR 1910.219(m)(1)(i): Guarding was not secured by the use of the following materials. Expanded metal, perforated or solid sheet metal, wire mesh on a fame of angle iron, or iron pipe securely fastened to floor or frame of machine. On or about May 22, 2018, and times prior, employees were operating a round downspout metal forming machine with a hinged guard that was not secured to prevent employee exposure to gears and rollers.
