29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.      The employer is failing to ensure employees are protected from fall hazards on site.  This was most recently documented at 904 Deer Run Lane Papillion, Nebraska on or about May 29, 2018 when the employer had employees working on the roof of this residence tying two employees to one anchorage point designed to hold the arrested weight of one person.  Employees are allowed  to overload the anchorage points while conducting installation of shingles as part of re-roofing operations.  More than one worker is being allowed to tie off to an anchorage point on the roof of the structure in which they were working on.  These anchor points are only designed for one person to be tied to them at a time.  The workers were exposed to falls of over 11 feet to the ground below.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:        The employer is failing to ensure employees are protected from fall hazards when performing roofing work. This was most recently documented at 904 Deer Run Lane Papillion, Nebraska on or about May 29, 2018 when the employer had employees working on the roof of this residence without adequate training on fall protection.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  The workers were exposed to falls of over 11 feet to dirt and concrete below.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     The employer is failing to provide employees with adequate and effective ladder safety training prior to allowing them to work on a residential style roof.  This was most recently documented at 904 Deer Run Lane, Papillion, Nebraska, on or about May 29, 2018, when the employer had employees access the residential roof with a ladder and perform re-roofing activities. Employees were not trained to recognize neither the hazards of ladders nor the proper procedures to be followed to minimize these hazards.  Workers were observed climbing the portable extension ladder, which was set up incorrectly.  The ladder stood less than three feet above the upper landing surface (roof) of the building and was not secured to prevent displacement. Workers were exposed to falls of over 11 feet to dirt and concrete below.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:     The employer is failing to provide employees with adequate and effective ladder safety training prior to allowing them to work on a residential style roof.  This was most recently documented at 904 Deer Run Lane Papillion, Nebraska on or about May 29, 2018  when the employer had employees working on the roof of the residence.  The workers were exposed to falls of over 11 feet to dirt and concrete below.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  Workers were observed climbing up portable extension ladders which were set up incorrectly.  Had the employees had this training the observed instances would not have been present on the job site.  The ladder did not stand three feet above the upper landing surface (roof) of the building nor was it secured from displacement.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident                                                                                                                          The employer failed to report an occupational fatality to OSHA within 8 hours of the death occurring.  This was most recently documented at 904 Deer Run Lane Papillion, Nebraska on or about May 29, 2018 when the employer had employees working on the roof of this residence when an employee of the company fell from a roof, was hospitalized and passed away four days later.  No notification of the death was made to OSHA by the employer.

This was most recently documented at 904 Deer Run Lane Papillion, Nebraska on or about May 29, 2018 when the employer had employees working on the roof of this residence when an employee of the company fell from a roof and was hospitalized.  No notification of the hospitalization was made to OSHA by the employer.
