29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) There was no program on energy control procedures, employee training, and periodic inspections, to include providing locks and training to employees rotating or changing the blades on the routers, or conducting weekly cleaning of the routers.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) On or about May 11, 2018, employees operating the four CNC routers were exposed to the unguarded blades.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) The employer did not report an employee's amputation to OSHA within 24 hours of the incident, in that on May 11, 2018, an employee suffered an amputation while operating a CNC router and the company did not the amputation to OSHA until May 16, 2018.
