29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  Florence Casket, 16 Bardwell St, Florence, Massachusetts; On and before June 6, 2018 the employer did not ensure the forklift operator was competent to drive a powered industrial truck safely through the successful completion of training and evaluation which includes formal training, practical training, and evaluation of the operator's performance in the workplace.

Florence Casket, 16 Bardwell St., Florence, Massachusetts; On and before April 24, 2018, the employer did not provide push sticks or push blocks in sizes and types suitable for use while employees performed surfacing of casket tops and bases.  One block was available, but it was not the proper size nor shape for the work being done.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    Florence Casket, 16 Bardwell St., Florence, Massachusetts; On or before April 24, 2018, the employer failed to report that an employee suffered a finger amputation injury while working on the Oliver 166-FD Jointer.  On May 24, 2018, the Springfield OSHA Office received a referral regarding the incident.
