29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a) Bagger 5 - Existing machine guards were not adequately designed to prevent employees from reaching inside the semi-automatic bagger machine through a 38 inch long x 8 ½ inch wide opening during the heat seal process. On or about May 24, 2018 an employee sustained crushing injuries when they reached inside the bagger to clear a jam.    No abatement certification or documentation required.
