Location - 6 Saddle Hill Road, Hopkinton, MA (house on the left) On or about May 31, 2018, employees were exposed to a fall hazard while working/walking on or near a wall opening and an unprotected second floor stairwell opening. Instance one. First floor wall opening located at the rear of the house. Wall opening was greater than ten feet above lower level. Instance two. No railing around stairwell opening on second floor. Second floor height was greater than ten feet above lower level.

Instance two: Employees were using an A-frame ladder to ascend and descend the basement from the first floor.
