29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   On or about June 1, 2018 the employer did not ensure that employees were protected from falling to ground level by any means. This condition exposed employee(s) to fall hazards above 6 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee�s knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:  On or about June 1, 2018 the employer did not ensure that employee(s) were adequately trained and that safe work practices were implemented when working from elevation. This condition exposed employee(s) to fall hazards above 6 feet.
