29 CFR 1926.451(g)(4)(xv): Cross bracing used in place of a mid rail was unacceptable because the crossing point of two braces was not between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a top rail crossing point of two braces were not between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.    (a) 1812 N. Lakewood Dr.: On or about May 30, 2018 and at times prior, at approximately 30 ft. employees were working on a scaffold with sections that did not have a top rail.  The 30 inch cross member met the height of a mid-rail, but it did not have a top rail.   (b) 1812 N. Lakewood Dr.: On or about May 30, 2018 and at times prior, at approximately  30 ft. employees were working on a scaffold with sections that did not have a mid rail.  The 39 inch cross member met the height of a top rail, but did not have a mid-rail.  Note: Abatement certification and documentation is required for this item.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below: Note:  This requirement does not apply to float (ship) scaffolds where an edging of 3/4 inch x 1 1/2 inch (2 x 4 cm) wood or equivalent was used in lieu of toe boards.  (a) 1812 N. Lakewood Dr.: On or about May 22, 2018 and at times prior, at approximately 30 ft. employees were installing brickwork from a scaffold and other contractors were working below at ground level.
