Employees were thereby exposed to struck-by, caught-in, and /or fall hazards from a height of approximately 27 feet. b. On or about June 04, 2018, at the above addressed inspection site, employees working in an aerial lift failed to conduct a pre-operational inspection of the boom in accordance with page 30 of the machine operators manual. Employees were thereby exposed to struck-by, caught-in, and /or fall hazards from a height of approximately 27 feet. There is no abatement certification of documentation required for this item.
