29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat. Location: Yard Employee was assisting in the emptying of hot thermoplastic road marking material from a truck kettle into a drum, and the employer had not taken adequate precautions to prevent the ignition of flammable vapors, including, but not limited to: taking precautions when there is only a small amount of thermoplastic material left in the pre-heating kettle, calibrating the kettle thermometer on a regular basis, and properly bonding the kettle and the drum together when transferring hot thermoplastic material from one to the other.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Location Yard Employee was assisting in the emptying of hot thermoplastic road marking material from the truck into a drum, and he received burns to his arms and face due to a sudden flash fire. He was not wearing proper protective equipment, including adequate heat-resistant gloves, long-sleeve shirt, and face shield. Hi-Way Safety Systems, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 986029, citation number 1 item number 2, and was affirmed as a final order on 06/12/15 with respect to a workplace located at 9 Rockview Way, Rockland, MA 02370.
