29 CFR 1910.134(e)(6): 29 CFR 1910.134(e)(6):  In determining the employee's ability to use a respirator, the employer shall obtain a written recommendation regarding the employee's ability to use the respirator from the PLHCP (physician or other licensed health care professional):    a) At the facility, on or about May 31, 2018 - Employees, who voluntarily wear a respirator, were not provided with a medical evaluation prior to donning a respirator during grinding operations.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  a) At the facility, on or about May 31, 2018 - Elements of a written respiratory protection program were not established when employees voluntarily wear respirators.  Employees wear a Gerson Half-Mask 8311P and a 3M Filtering Facepiece respirator during grinding operations.
