29 CFR 1926.1053(a)(7): Ladders not specifically designed for such use were tied or fastened together by the employer to provide longer sections: a) tellospainting at 454 Paintbrush Lane, Louisville, CO 80027: On or before June 4, 2018, the creating and exposing employer used an "a" ladder tied to a "b" ladder to reach three stories by tying or lashing together with rope and nylon webbing, attaching a ten foot ladder to a 32 foot extension ladder. A worker was observed using this ladder. Injuries associated with falls of approximately 30 feet are, but are not limited to; head injuries; leg and ankle fractures, muscle and ligament damage, lacerations, and full body contusions. The most serious injury as a result of a fall from an elevation of 30 feet is permanent disability or death.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: a) tellospainting at 454 Paintbrush Lane, Louisville, CO 80027: On or before June 4, 2018, the employer place two ladders in the work area. One ladder to the north was tied together, had one broken foot, and was placed on the sidewalk without any barricade to prevent displacement. The second ladder to the south had two broken feet and was placed in the soil without any barricade to prevent displacement. Both ladder had broken feet which would allow the ladder to slip and fall out from underneath the workers. Injuries associated with falls of approximately 30 feet are, but are not limited to; head injuries; leg and ankle fractures, muscle and ligament damage, lacerations, and full body contusions. The most serious injury as a result of a fall from an elevation of 30 feet is permanent disability or death.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: a) tellospainting at 454 Paintbrush Lane, Louisville, CO 80027: On or before June 4, 2018, the employer placed two ladders in the work area. One ladder to the north was tied together, had one broken foot, and was placed on the sidewalk without any barricade to prevent displacement. The second ladder to the south had two broken feet and was placed in the soil without any barricade to prevent displacement. Both ladder had broken feet which would allow the ladder to slip and fall out from underneath the workers. No ladders in the worksite were tagged out with "Do Not Use" nor was there a process to inspect and tag out ladders that were broken. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. a) tellospainting at 454 Paintbrush Lane, Louisville, CO 80027: On or before June 4, 2018, the employer placed two ladders in the work area. One ladder to the north was tied together, had one broken foot, and was placed on the sidewalk without any barricade to prevent displacement. The second ladder to the south had two broken feet and was placed in the soil without any barricade to prevent displacement. Both ladder had broken feet which would allow the ladder to slip and fall out from underneath the workers. The workers were not trained to recognize and minimize the hazards of using ladders or how to handle broken ladders or why ladder should never be tied together. Injuries associated with falls of approximately 30 feet are, but are not limited to; head injuries; leg and ankle fractures, muscle and ligament damage, lacerations, and full body contusions. The most serious injury as a result of a fall from an elevation of 30 feet is permanent disability or death.
