OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a) White Mountain Operating LLC at RSU Anschutz Fed 4-62-02-4841 CS in Greeley, CO 80634:  On or before June 1, 2018 the employer did not ensure that employees were protected from hazards such as those created by ingoing nip points and rotating parts.  Employees working on the rig floor of the Schramm Model T130XD during casing operations were exposed to amputation hazards caused by the lack of guarding.  The rollers running vertically along the rig rails used in moving the top drive were not guarded on the off-driller side.   An employee working on the rig floor experienced a partial amputation of two fingers when a roller ran over his fingers.  This condition exposed employees to amputation hazards.
