29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):     a. 154 Industrial Park Circle, Lawrenceville, GA (Jet Machines #1) - Employees were exposed to struck-by hazard.  The employer did not  provide machine guarding and/or barriers on the water jet machine when making regular 2-axis cuts and/or 5-axis cuts to the foam.  The light curtain was not operable when making the 5-axis cuts. There is no guarding to prevent the employees from coming in contact to the movable water jet head where the high pressure waters dispenses out to cut the foam, exposing employees to injuries, on or about 06/05/18.      Atlanta Foam Fabricators, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.212(a)(3)(ii), which was contained in OSHA inspection number 1296238, citation number 1, item number 1 and was affirmed as a final order on April 27, 2018, with respect to a workplace at 154 Industrial Park Circle, Lawrenceville, GA.
