OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING CAUGHT-IN and BY BEING STRUCK BY the load and/or lift components should the lift fail. a) At the facility, on or about 7/26/18: The operator of a 4 post automatic lift was exposed to a CAUGHT-IN hazard by manually disengaging the safety locks during the downward direction of the lift and not disengaging the safety locks using the required pneumatic device as originally equipped by the manufacturer. b) At the facility, on or about 6/5/18: The operator of a 4 post automotive lift was exposed to a STRUCK-BY hazard from a falling load while manually disengaging the safety locks during the downward direction of the lift and not disengaging the safety locks using the required pneumatic device as originally equipped by the manufacturer. Abatement documentation must be submitted for this item.
