29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards. a. On or about June 5, 2018, at the workplace located near the auger welding department, the employer failed to secure three propylene cylinders to prevent the bottles from being knocked over and/or damaged, thereby exposing the employees to a struck-by hazard.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. a. On or about June 5, 2018, at the workplace located near the auger department, oxygen cylinders were stored together with propylene cylinders, thereby exposing the employees to a greater hazard if a fire would occur.

29 CFR 1910.179(g)(1)(v): Pendant control boxes shall be constructed to prevent electrical shock and shall be clearly marked for identification of functions. a. On or about June 5, 2018, at the workplace located near the auger department, employees operated crane number A24, a CRB 15 ton overhead crane that had a pendant control box with the up and down buttons not marked to identify the functions for moving fabricated steel products, thereby exposing employees to a struck-by hazard.
