29 CFR 1910.145(c)(3): Safety instruction signs. Safety instruction signs were not used where there was a need for general instructions and suggestions relative to safety measures. On or about May 26, 2018, the employer did not ensure well control was maintained while high pressure lines were being moved from one well to another at a multi-well pad site during fracturing operations. Employees were exposed to struck-by hazards.
