29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) This violation occurred on June 5, 2018 in the planer mill of the facility located at 14732 State Highway 87 North, Timpson, Texas; where an employee operating a Hyster (model: 100 Fortis forklift) without wearing a seatbelt was exposed to a crushed by hazard in the event of a tip over;    b) This violation occurred on June 5, 2018 in the planer mill of the facility located at 14732 State Highway 87 North, Timpson, Texas; where an employee was operating a Taylor (model: THD160) forklift with a broken seatbelt was exposed to a crushed by hazard in the event of a tip over.

29 CFR 1910.28(b)(15): Walking-working surfaces not otherwise addressed. Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walking-working surface 4 feet (1.2m) or more above a lower level was protected from falling by: (i) Guardrail systems; (ii) Safety net systems; or (iii) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems:    (a) This violation occurred on June 5, 2018 on top of the sorting machine near drop position 44 where employees working on fixing the far conveyor chain that had come off its rail due to a product jam because of a malfunctioning diverter were exposed to a fall hazard of up to 10 feet to the concrete floor below.

29 CFR 1910.147(c)(2)(ii): If an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section shall utilize lockout, unless the employer can demonstrate that the utilization of a tagout system will provide full employee protection as set forth in paragraph (c)(3) of this section:      a) This violation occurred in the planer mill at the HEMCO sorter system where employees working on top of the sorter machine when repairing a chain that had come off of its track and removing jammed lumber without locking out the unit were exposed to a caught-in hazard.
