Wood Shop, on or about 05 June 2018 and times prior thereto; a Rockwell Arbor Saw, model 34-395-313, serial number 1756928, did not have the splitter and guard assembly installed. This condition exposed employees to a caught in hazard.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. Auto Shop, on or about 05 June 2018 and times prior thereto, a Van Dorn 10-inch ball bearing bench grinder, serial number 259327; tool rest was not properly adjusted to a maximum distance of one-eighth of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured approximately three-quarters of an inch. This condition exposed employees to a caught in hazard.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. Auto Shop, on or about 05 June 2018 and times prior thereto, a Van Dorn 10-inch ball bearing bench grinder, serial number 259327; tongue guard distance was greater than 1/4 of an inch from the abrasive wheel. Tongue guard was approximately one-inch from the abrasive wheel. This condition exposed employees to a caught in hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Automotive Shop, on or about 05 June 2018 and times prior thereto; a 50-foot orange extension cord was missing the ground pin. This condition exposed employees to an electrical shock and electrical burn hazard.
