The switch was energized and powering the ceiling light fixture.    b) A 4 inch junction box being used as housing of a receptacle was not installed with a cover.  The receptacle was being used to power an extension cord.    c) A 4 inch junction box housing a receptacle connected to a flexible cord was being used as an extension cord.  The  receptacle was not provided with a faceplate.  It was being used to power a battery charger.
