Location: 821 45th Street, Brooklyn, NY; Basement a) A 4 inch junction box housing a light switch in an energized system was not mounted. It was used to power the ceiling light fixture in the room. b) A 4 inch junction box housing a receptacle in an energized system was not mounted. It was used to power an extension cord. c) A 4 inch junction box housing a receptacle was connected to a flexible cord and was not mounted. It was placed loosely on the floor and being used to energize a battery charger.

Location: 821 45th Street, Brooklyn, NY; Basement a) A 4 inch junction box also being used as housing of a light switch was not installed with a cover. The switch was energized and powering the ceiling light fixture. b) A 4 inch junction box being used as housing of a receptacle was not installed with a cover. The receptacle was being used to power an extension cord. c) A 4 inch junction box housing a receptacle connected to a flexible cord was being used as an extension cord. The receptacle was not provided with a faceplate. It was being used to power a battery charger.
