29 CFR 1910.134(h)(1): Cleaning and disinfecting. The employer shall provide each respirator user with a respirator that is clean, sanitary, and in good working order. The employer shall ensure that respirators are cleaned and disinfected using the procedures in Appendix B-2 of this section, or procedures recommended by the respirator manufacturer, provided that such procedures are of equivalent effectiveness. The respirators shall be cleaned and disinfected at the following intervals: a. On or about June 6, 2018, employees in the paint room were provided with full face respirators, which they stored on a cabinet at the end of the shift without cleaning/disinfecting. The employer did not ensure that the respirators used by the paint room employees were cleaned/disinfected prior to storage at the end of the shift.

29 CFR 1910.134(h)(2)(i): All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve. a. On or about June 6, 2018, employees in the paint room were provided with full face respirators, which they left on top of a cabinet with multiple other items at the end of the shift. The employer did not ensure that the respirators used by the powder coating area employees were properly stored at the end of the shift to prevent damage and or deformation.

29 CFR 1910.134(k)(1)(v): What the procedures are for maintenance and storage of the respirator; a. On or about June 6, 2018, in the paint room, employees who were provided with full face respirators left the respirators laying dirty, uncovered and unlabeled on a cabinet in a small room at the rear of the paint room. The employer had failed to adequately train the paint room employees about the correct procedures for cleaning and storage of the respirators.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. a. On or about June 6, 2018, employees in the paint room of the work site used paints that contained eye irritants that included, but were not limited to, barium sulfate, copper dust and mist, crystalline silica dust, titanium dioxide dust, methyl isobutyl ketone mist, ethylene glycol mist and ethylene glycol monobutyl ether mist. The employer had not provided employees with immediate access to a method for flushing of the eyes in the paint room area.
