OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and caught in between hazards: a) On or before May 31, 2018, at the jobsite located at 10000 W. O'Hare Ave., Chicago, IL, the employer utilized a Sandvik QJ341 mobile jaw crusher as part of the O'Hare Airport expansion project. Employees were positioned on the maintenance platform of the crusher while it was in operation, which exposed them to being struck by material as it was dropped into the hopper and/or if pieces of material were ejected by the machine. Feasible and acceptable methods of abatement to correct this hazard is to prevent employees from standing on the crusher platforms or within the exclusion zones while the equipment is in operation, as specified in the Sandvik QJ341 operator's manual.
