29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Shop - On or about 06/06/18 and at times prior thereto employees handled and applied Wire & Mag Wheel Cleaner (AIM, Auto, Industrial, Marine Chemicals, Inc.) to clean boat parts. The label indicated"corrosive" containing hydrofluoric acid, and the employer did not provide an eye and body drenching/flushing equipment in case of accidental contact with the chemical.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturer's prior written approval:  a) Facility - On or about 06/06/18 and at times prior thereto the employer modified the original Diesel-powered Clark forklift without manufacturer's approval and utilized fork attachments to extend the length of the forks for the elevation of pontoon boats.     Or in the alternative:   29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, and were altered either by the addition of extra parts not provided by the manufacturer or by the elimination of parts:  a) Facility - On or about 06/06/18 and at times prior thereto the employer modified the original Diesel-powered Clark forklift without manufacturer's approval and utilized fork attachments to extend the length of the forks for the elevation of pontoon boats.
