When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided:    a) Facility, 555 South Main Street, Harrisville, PA:  On or about June 5, 2018, employees working in the washer room are exposed to wet floor conditions due to the Milnor washer #4 that water has been flowing from the side loading door for the past year.

29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    a) Facility, 555 South Main Street, Harrisville, PA:  On or about June 5, 2018, the employer provided lockout tagout procedures for employees to utilize, but none of the 62 procedures in the program were titled/labeled to be designated for each specific machine.
