29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use did not present a health hazard to the user    a) Facility: The employer did not establish and implement a written respiratory program for employees  that wore 3M half mask respirators voluntarily, on or about 6/6/18.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a) Manufacturing: The employer did not provide a medical evaluation to employees that wore a 3M half mask respirator voluntarily to perform job duties such as, but not limited to cutting card board boxes with the band saw and sweeping the facility, on or about 6/6/18.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.134(h)(2)(i): The employer did not ensure that respirators were stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the face piece and exhalation valve:  a) Manufacturing (Baler): The employer did not ensure that employee's 3M half mask respirators were stored properly to protect from dust, on or about 6/6/18.  b) Manufacturing (Band Saw): The employer did not ensure that employee's 3M half mask respirators were stored properly to protect from dust, on or about 6/6/18.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
