29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: a) At the time of inspection the employer failed to provide written verification of a workplace hazard assessment. A written certification is one that identifies; the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment. Employees operating milling machines throughout the facility were thereby potentially exposed to hazards associated with milling processes.
