M-19 13 did not use energy control procedures and were exposed to an amputation hazard.

29 CFR 1910.212(a)(1): One or more machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying, chips and sparks: (a) This violation occurred on or about May 31, 2018, at a workplace located at 5710 Reed Road, Tyler, TX, where employees working in the production area on the Universal Eccentric Vertical Slotter, serial no. M-19 13 were exposed to an amputation hazard due to a guard with an opening of approximately 2-inches.
