29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 dBA:    a)  At the facility, on or about June 20, 2018 - A continuing, effective hearing conservation program was not instituted when employee noise exposures equaled or exceeded an 8-hour time weighted average of 85 dBA.  An employee, designated as a Screw Machinist, had an exposure to continuous noise at a dose of 68.49%.  The exposure is approximately 1.38 times the OSHA action level of 85 dBA.  This is equivalent sound level of 87.2 dBA.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  a)  At the screw machine, on or about June 20, 2018 - An audiometric testing program was not provided to employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels.  An employee, designated as a Screw Machinist, had an exposure to continuous noise at a dose of 68.49%.  The exposure is approximately 1.38 times the OSHA action level of 85 dBA. The employer did not institute a training program and ensure employee participation in the program:   a)  At the screw machine, on or about June 20, 2018 - Training was not provided to each employee whose exposure to noise was at or above an 8-hour time-weighted average of 85 decibels.  An employee, designated as a Screw Machinist, had an exposure to continuous noise at a dose of 68.49%.  The exposure is approximately 1.38 times the OSHA action level of 85 dBA.  This is equivalent sound level of 87.2 dBA.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a)  At the facility, on or about June 20, 2018 - Employees were not provided with training on the workplace labeling system and safety data sheets.  Employees are exposed to Hangsterfer's C22, an irritant, during machining operations.
