29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about June 4, 2018, at 3550 Flight Safety Dr., Vero Beach, FL 32960, a steel beam that was not secured to prevent from shifting was being carried on the forks of a powered industrial truck that was operated on un-level ground, thus exposing an employee to a crushing hazard.
