On or about June 5, 2018 and there prior to, the employer did not have a lockout tagout procedure for equipment such as but not limited to industrial sewing machine automatic feeder, model 1000. This condition exposed employees to caught-in hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about June 5, 2018 and there prior to, the employer did not provide any type of machine guarding for conveyor belts and pulleys on the sewing line automatic feeders, model 1000. This condition exposed employees to pinch points and caught-in hazards.
