29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: The employer failed to determine and document that the anhydrous ammonia refrigeration system was adequately designed, installed, and maintained to protect employees from an uncontrolled release of anhydrous ammonia in compliance with recognized and generally accepted good engineering practices (RAGAGEP). b) Nation Pizza Products, L.P.: On June 7, 2018 the employer did not ensure that the design calculations and/or verification of the current fan design, for the mechanical ventilation system, took into account conditions such flammable storage cabinets and materials stored in front of intake louvers in the engine room 2 as stipulated in RAGAGEP such as ANSI/IIAR Standard 15. c) Nation Pizza Products, L.P.: On June 7, 2018 the employer did not ensure that a safety shower and eye wash station was installed external to engine room 2 in accordance with IIAR-2.
