29 CFR 1910.269(o)(6)(ii): The test operator in charge shall conduct routine safety checks before each series of tests and shall verify the conditions: Albany Substation - On or about 04 June 2018, the employer did not ensure that an adequate safety check was conducted to ensure that an employee exposed to contact with 10,000 Volts (10kV) during power factor testing on a substation transformer, was required to use personal protective equipment, including voltage-rated rubber insulating gloves. An employee was exposed to shock/electrocution. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
