29 CFR 1926.304(i)(1): Each circular hand-fed rip saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut: At the jobsite located at 8115 Lee Blvd. in Leawood, Kansas. The 10 DeWALT table saw was missing the point of operation guard over the saw blade, exposing the employees to amputation hazards.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: At the jobsite located at 8115 Lee Blvd. in Leawood, Kansas. The employer failed to call OSHA to report an incident within 24 hours after the employee was injured. The employee was exposed to an amputation hazard that resulted in the amputation of four fingers on the left hand.
