Employees were exposed to a potential fall distance of 20 feet.  b) On or about June 5, 2018 or prior thereto:   Fall protection was not utilized during installation of floor hole covers on the third floor deck of the Triplex being constructed.  Employees were exposed to a floor hole of approximately 3 feet by 4 feet, and a potential fall distance of 20 feet.  NOTE:   Abatement certification is NOT required for this item.
