Haddon Ave, in Haddonfield, NJ, 08033, on the rooftop of the upper roof (second story) on the south side of the building: Employees were exposed to fall hazards when they performed a roof leak repair and replacement at approximately 18 feet without any means of fall protection that could result in death, which occurred and was observed on or about 5/21/18.   b) Mixed use commercial/ residential property at 134 N. Haddon Ave, in Haddonfield, NJ, 08033, on the rooftop of the lower roof on the north side of the building: Employees were exposed to fall hazards when they performed accessed the upper roof via the lower roof at approximately 12 feet without any means of fall protection that could result in death, which occurred and was observed on or about 5/21/18.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
