OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees who operated a golf cart/transport vehicle were exposed to fall/struck-by hazards due to missing hand holds and hip restraints: a) On or about 06/01/2018, at and between the main building and the resort building, employees used golf carts/transport vehicles to move parts and personnel. An EZ-Go golf cart/transport vehicle used for personnel and parts transport, serial number 577556 and Manufacturer Code B2390, was operated without hand holds and hip restraints resulting in employees being exposed to fall and struck-by hazards.

The work rest was adjusted 2/8 of an inch from the abrasive wheel.
