29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:    (a) 587 Laurel Lane, Lancaster, Pennsylvania - The employer did not have a competent person involved in the job site in that none of the employees who worked on the site on June 8, 2018 inspected the job site, identified predictable fall hazards, and took immediate actions to correct the hazards.  As a result, employees did residential roofing work without using any fall protection system, and employees used portable extension ladders that did not extend three feet above the landing surface to access the roof, exposing employees to the hazard of falling to a lower level, on or about June 8, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    (a) Roof - Employees worked removing the old roofing and installing new roofing on the two-story single-family home and on the one-and-a-half-story attached garage.  The employees did not use any fall protection, exposing them to the hazard of falling from the roof to the ground below, on or about June 8, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) Roof over Garage - Two employees used a portable extension ladder to gain access to the roof to tear off the old roofing.  This ladder did not extend three feet above the roof (upper landing) surface, exposing the employees to the hazard of falling about 9 feet to the ground below, on or about June 8, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
