29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. (a) on the jobsite, south east corner - the portable extension ladder that was extended from the ground up to the 3rd buck did not provide access to the platform of the 4th buck, exposing employees to a 13ft,9.5in up to 20ft,3in fall hazard; observed on or about 6/8/2018.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) on the jobsite, south east corner - the portable extension ladder did not extend 3 feet above the platform of the 3rd buck of the tubular welded frame scaffold, exposing employees to a 13ft,9.5in fall hazard; observed on or about 6/8/2018.

(b) eastern side of building - missing mid-rail at the southern section of the 4th buck, exposing employees to a 20ft,3in fall hazard; observed on or about 6/8/2018. (c) eastern side of building - missing guardrails at the southern section of the 3rd buck where the top of the access ladder was located, exposing employees to a 13ft,9.5in fall hazard; observed on or about 6/8/2018. (d) eastern side of building - missing mid-rails at the middle section and northern section of the 3rd buck, exposing employees to a 13ft,9.5in fall hazard; observed on or about 6/8/2018. (e) eastern side of building - missing end guardrail at the north end of the 3rd buck, exposing employees to a 13ft,9.5in fall hazard; observed on or about 6/8/2018.
