29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support:  a)  Route 873, Slatington - On or about June 11, 2018, aluminum extension ladders were placed at a 60 and 65 degree angle such that the horizontal distance from the top support to the foot of the ladder exceeded one-quarter of the working length of the ladder.

29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.  a)  Route 873, Slatington -  The employer does not ensure that appropriate eye personal protective equipment or methods are in use. A specific instance of this continuing violation was most recently observed on or about June 11, 2018, when employees were performing residential roofing activities while using pneumatic nail guns without wearing impact resistant safety eyewear. The employer's ongoing obligation is to ensure that the proper safety eyewear is worn during pneumatic nail gun usage.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a)  Route 873, Slatington - The employer does not ensure that appropriate fall protection equipment or methods are in use. A specific instance of this continuing violation was most recently observed on or about June 11, 2018, when employees were performing residential roofing activities while working without fall protection at the unprotected edges of a steeply pitched roof, approximately ten to 25 feet above a lower level. The employer's ongoing obligation is to ensure that fall protection equipment and/or methods are in use when employees are exposed to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  Route 873, Slatington - The employer does not ensure that portable extension ladders are being used properly. A specific instance of this continuing violation was most recently observed on or about June 11, 2018, when employees were performing residential roofing activities using an extension ladder that was not extended at least three feet above the upper landing surface.  Alternatively, the ladder was not secure, nor had grasping devices available for the employee's use. The employer's ongoing obligation is to ensure that portable ladders are set for the proper working length.
