29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) 340 North 2nd Street Chambersburg, PA - An employee utilized a JLG 450-A aerial lift to perform elevated work activities, chiseling out block, and was not protected/restrained inside the basket with a fall protection harness and lanyard, exposing the employee to a possible ejection and fall hazard of approximately 20 feet, on or about May 29, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
