The hallway exits access from north side of the production room was 24.3 inches wide at two points near block structural supports.     b) On June 11, 2018 Grippo Potato Chip Company, Inc. did not ensure that all exit accesses were at least 28 inches wide at all points. The hallway exits access South of the Lunch room  room was 25 inches wide at points near block structural supports.

An industrial Maxx Air Pro fan with was powered with an orange extension cord that had been wrapped up and around the metal conduct and I-beams from the outlet.
