29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) Les Schwab Tire Center of Colorado Located at 7005 West 10th Street, Greeley, CO 80634: On and Preceding 12 June, 2018, The employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks in the machine area of an Ammco Model 6950 Brake lathe. Employees use the brake lathe up to 20 times per week to mill down brake rotors and drums without the guard installed. This exposes employees to caught-in hazards.

29 CFR 1910.28(b)(8)(ii): (ii) Applies floor markings at least 6 feet (1.8 m) from the edge of the pit in colors that contrast with the surrounding area; or places a warning line at least 6 feet (1.8 m) from the edge of the pit as well as stanchions that are capable of resisting, without tipping over, a force of at least 16 pounds (71 N) applied horizontally against the stanchion at a height of 30 inches (76 cm); or places a combination of floor markings and warning lines at least 6 feet (1.8 m) from the edge of the pit; and a) Les Schwab Tire Center of Colorado Located at 7005 West 10th Street, Greeley, CO 80634: On and Preceding 12 June, 2018, The employer did not apply floor markings at least 6 feet from the edge of the pit in colors that contrast with the surrounding area; or place a warning line at least 6 feet from the edge of the pit as well as stanchions that are capable of resisting, without tipping over, a force of at least 16 pounds applied horizontally against the stanchion at a height of 30 inches or place a combination of floor markings and warning lines at least 6 feet from the edge of the pit. The repair pit was 6' deep and had an open sided section which was marked on the edge with caution tape, a yellow chain and a sign warning of the pit. employees walk back and forth near the edge several times per day while performing in shop maintenance. This exposed employees to falls from height.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard: a) Les Schwab Tire Center of Colorado Located at 7005 West 10th Street, Greeley, CO 80634: On and Preceding 12 June, 2018, The employer Ensure that the stairways leading into the alignment pit were protected on the open side by a stair rail system. The staircases on each side of the alignment pit had 5 treads, 6 risers and measures over 44 inches in width with one side open and on side enclosed. The side opening into the pit on both staircases had the stair rail removed. Employees use the stairs to access the pit several times per day while performing in shop maintenance. This exposed employees to falls from height into the pit.
