On or about June 12, 2018, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about June 12, 2018, at the above addressed jobsite, employees performing residential roofing operations on a roof approximately (28) feet above the ground were not protected from falls, thereby exposing employees to fall hazards. Quality Construction Pros Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1303591, citation number 1, item number 3 and was affirmed as a final order on May 31, 2018, with respect to a workplace located at 5820 S. Nordica Ave, Chicago, IL 60638.
