Sycamore, Genoa, IL - On or about June 12, 2018, the exit route for the exit door on the east side of the building was obstructed by warehouse product, exposing employees to delayed egress and burn/smoke inhalation hazards in the event of a fire. There is no abatement documentation or certification required for this item.

29 CFR 1910.178(p)(1): Powered industrial trucks found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition. Warehouse Bldg., 538 S. Sycamore, Genoa, IL - On or about June 12, 2018, the following powered industrial trucks had horns that did not work, exposing employees to struck by hazards. The vehicles were not removed from service until repaired. a. Green Clark No. 4, propane sit-down truck - the horn was taped over and did not work. b. Orange Toyota "Atlas", propane sit-down truck - the horn did not work. There is no documentation or certification required for this item.
