The employer did not ensure employees were protected from falling while walking down a set of stairs 7 feet above the ground while holding and pulling on the rope to raise that aluminum walkway.

29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower level: On or before June 12, 2018, employees were exposed to a 6 inch gap approximately 24 feet long and 7 feet above the ground. The gap was between the open sided edge of an elevated walkway and the side of the staged truck trailer in the roofing department at the roof installation area. The employer did not protect employees from the fall hazard.
